24 x 7 EMERGENCY NUMBER
 
All Cirrus Medical Staffing employees are privy to an emergency phone number to be used ONLY for emergencies.  This gives you 24 hour access to a Cirrus employee (opposed to an answering service) in the event you need us for emergency situations.
401(k) PLAN
 
Cirrus employees may join our 401(k) retirement plan immediately OR at anytime during their employment.  It takes 30 days to process enrollment.  Our investments are chosen by Merrill Lynch and pay the highest yields based on past performance.  Regardless of your investments selected as conservative, moderate or aggressive, your return is maximized. 

Cirrus Medical Staffing will match 100% of the first 2% of your PAY (vs. 2% of your contribution).  Vesting will be at 50% after one year; 75% after two years and 100% after 3 years of employment.  You must work a minimum of 1,000 hours to enter the vesting program.

BONUS PROGRAMS
 
Cirrus Medical Staffing offers eight different Bonus Programs.  Bonuses are rewards for a number of outstanding qualities that we value in our employees such as longevity, attendance, referrals of other travelers, paperwork compliance, timely updates required yearly and others.  Chances are, there is a bonus with your name attached to it!
COMPENSATION
 
The Travelers of Cirrus Medical Staffing are paid on a WEEKLY basis, provided the travelers timesheet is received on time (Mondays by noon EST).  Pay is provided via direct deposit. 

Alternatively, for those who prefer not to have a bank account, Cirrus is positioned to provide (for a small initiation fee of less than $5.00) a Traveler Access Card, called an ADP TotalPay Card, (much like an ATM/Debit card) that can be used just like a bank account and is accepted throughout the enitre United States.

FURNITURE SELECTION PROGRAM
 

This program allows our travelers to upgrade furniture packages at a cost and also allows the purchase of "soft packages" if desired.  All furniture packages, including the furniture that Cirrus provides at no cost, is available for view along with monthly pricing for upgrades.  This is posted in the FORMS section of our website.


HOLIDAY PAY
 
Holiday Pay is provided for holidays observed at your assignment facilities.  Such holidays and the accelerated pay for working the observed holidays are outlined in detail on your contract.
HOUSING
 
Private housing is provided for by Cirrus Medical Staffing.  Our standard housing package consists of a one bedroom apartment with a furnished den, kitchenette and bedroom. Cirrus selects it's housing based on: 1) Safety, 2) Cleanliness and 3) Proximity to facility.  Cirrus will put all utilities in our name and pay up to $75.00 monthly toward your utility usage.  Telephone, cable or soft packages (dishes, towels, etc.) are the responsibility of the traveler.  Additional arrangements can also be made at cost (additional bedrooms, pets, etc.).

Alternatively, Cirrus will pay a monthly housing stipend, for which the traveler can locate their own housing.  Please ask your recruiter about details pertaining to housing stipends.

At Cirrus, you are welcomed to travel with family members, companions and pets as long as you notify us in advance.

IDENTITY THEFT PROTECTION PROGRAM
 
According to the Federal Trade Commission, 1 of 3 Americans will have their Identity stolen this year.  Protect yourself (including your spouse) for a fee of less than $15.00 per month.  This is a protection service that any Cirrus Traveler may continue on a month to month basis.  It is a service that is an elective service by the Cirrus Traveler.  If you enroll, the coverage is for you (and your spouse, if you choose, at no additional cost).  This service is available to all travelers regardless of your location within the entire United States and the four provinces of Canada.

LIABILITY INSURANCE
 
As required by law, Cirrus provides ample liability insurance coverage for each traveler on our payroll, at no cost to the traveler.
LICENSE REIMBURSEMENT
 
Cirrus Medical Staffing will reimburse you up to $100 toward obtaining licensure needed to practice in a state outside of your state of origin.
LONG-TERM CARE INSURANCE
 
Offered by Mutual of Omaha, Long-term care insurance allows you or your loved ones to secure  quality helathcare at either a skilled nursing facility OR at home.  Long-term care coverage allows you to protect your savings and preserve important relationships with additional family members.  You may purchase Long-Term Care Insurance for yourself OR any of your family members.

MAJOR MEDICAL INSURANCE BENEFITS
 
Cirrus provides our employees with PAID major medical insurance coverage through CIGNA, effective day one of employment.  CIGNA provides NATIONAL COVERAGE and has providers throughout all 50 states.  This is a premium plan called Open Access Plus, a PPO Plan designed for major coverage and provides comprehensive coverage. 

Additionally, you may, for a fee, also elect to provide coverage for:

Spouse only
Domestic Partner only
Child(ren) only
Family

Vision Insurance is included in the Enhanced Plan.  You may elect to upgrade your benefits to provide addtional coverage, including lower deductables, lower co-pays and include vision insurance for a monthly fee (Enhanced Plan).

COBRA INSURANCE , as required by law, will be offered to all exiting employees of Cirrus Medical Staffing.  You will receive your COBRA option notification within 14 days of your exit date.  The name of the Cirrus administrator organization for COBRA is Ceridian.  Once you recieve your COBRA information in the mail, you will have 60 days to make a decision and up to 45 days more to submit the enrollment forms and a check to Ceridian in order to become covered with insurance coverage under COBRA guidelines.  Please be aware that the money due to Ceridian will be retroactive, monthly, from your exit date to present date for insurance coverage under COBRA laws.

ON-LINE FORMS
 
Forms for employment and required yearly updates are located on-line through the Cirrus website, www.cirrusmedicalstaffing.com . You merely print, read, sign and fax back to us (all Cirrus faxes are operataive 24 x 7).  Cirrus travelers are not permitted to start an assignment unless all required paperwork is received prior to your assignment start date.  Upon hire, a Cirrus Quality Control Sepcialist will contact you with deadline dates to have your paperwork completed and received at Cirrus.  Your Quality Control Specialist is responsible for seeing that your assignment starts troublefree with the client facility of your choice.
ON-LINE PAYROLL STUBS
 
You have 24 hour access to your payroll stubs via the Internet.  You are provided with instructions on the operatins of this service upon hire.
PAID TIME OFF PROGRAM (PTO)
 
This program is NOT associated with our Vacation Pay Program (two separate benefits).  This program rewards working beyond the call of duty.  Paid Time Off certificates are earned by working blocks of hours of overtime.  Once you meet a certain block of overtime hours worked, you are issued a certificate for a paid day off.  You can earn up to three paid days off for working an accrued number of overtime hours in a year.
PROFESSIONAL LIABILITY INSURANCE
 
At no cost to you, Cirrus maintains your Professional Liabililty Insurance, protecting your licensure and right to practice in your field.  Our coverage exceeds the standard coverage provided by most firms.  We take the protection of your professional status seriously.
SCHOLARSHIP PROGRAM
 
Cirrus Medical Staffing offers a yearly $1,000.00 Scholarship, which is open to ALL Cirrus travelers and their immediate family members to compete.  Healthcare focus IS NOT a requirement to participate in the competition for the Scholarship award.  You must be actively employed by Cirrus Medical Staffing to participate.  You can obtain detailed information by contacting Greg Allen, President at gallen@cirrusmedicalstaffing.com OR you can refer to the Employee Handbook on-line for details.
SUPPLEMENTAL INSURANCE CHOICES
 

In addition to your paid insurance benefits, Cirrus Medical Staffing also offers additional options for insurance coverage for cost.  Through Colonial Life Insurance Company, you may also elect to pay a modest monthly rate for any of the following additional coverage plans:

Short Term Disability
Long Term Disability
Cancer Insurance
Accident Insurance

Note:  These options are in ADDITION to the paid coverage provided in your major medical coverage (for those who wish to carry more insurance than your paid  insurance plan offers).  Cancer, Accident and Life insurance is already included in your paid insurance coverage; however, options to increase such specific coverage at a low cost is your choice

TAX ADVANTAGE PROGRAM
 
The IRS allows travelers certain tax free "per diems" for meals and incidentals, which apply to traveling professionals.  Cirrus Medical Staffing uses the approved IRS guidelines for all travelers who choose to take advantage of this program.  We protect your best interests to ensure that this benefit is applied properly so that you aren't penalized on your yearly income taxes.

Tax Free housing "per diems" are also available; however, due to IRS rules, companies, such as Cirrus Medical Staffing, are not permitted to provide housing OR a housing stipend ALONG with a housing "per diem" that is approved by the IRS.

TRAVEL PAY
 
Cirrus pays travel pay to and from the assignment, up to $500 per assignment.

VACATION PAY
 
This program is NOT associated with Cirrus' Paid Time Off Program (two separate benefits).  After four consecuitve 13 week assignments, you qualify for Vacation Pay.  Pay earned for vacation is paid at the average hours of workweek worked between all assignments at the average pay rate paid between all assignments.

WORKERS COMPENSATION INSURANCE
 
As required by law, Cirrus provides paid workers compensation insurance on each traveler.  Should be become injured on the job, please complete an incident report (contact your recrutier for this form) and provide all details in the documentation.  You must turn this form in within 24 hours of the occurance to ensure that you are covered under workers compensation insurance.  Please also refer to procedures in your employee handbook, located online.
